This Photo Release Form covers both adult and child likenesses that appear in your photo. It is the photographer's responsibility to complete a form when a person's likeness is used in your photograph. You need to complete a separate form for each likeness in your photograph. Your release form(s) must be submitted along with the image file. Any photograph received where an individual's likeness is promently displayed, but no photo release form was received for the photo, the photo will be disqualified.
